The Finnish Data That Convinced Cardiologists
Finland has one of the highest rates of sauna use in the world — roughly three million saunas for a population of five and a half million people. It also, not coincidentally, became the source of some of the most compelling long-term data on sauna use and cardiovascular health ever collected.
The Kuopio Ischemic Heart Disease Risk Factor Study followed 2,315 Finnish men between the ages of 42 and 60 for an average of 20 years, documenting their sauna habits alongside a comprehensive range of cardiovascular risk markers. The findings, published in JAMA Internal Medicine in 2015, were striking enough to draw international attention. Men who used the sauna two to three times per week had a 27 percent lower risk of fatal cardiovascular disease events compared to those who used it once a week. Men who used it four to seven times per week had a 50 percent lower risk. All-cause mortality showed a similar dose-response relationship.
The Cardiovascular Mechanism
A typical Finnish sauna session — 80 to 100°C for 15 to 20 minutes — produces a cardiovascular response that resembles moderate aerobic exercise. Heart rate increases to 120 to 150 beats per minute. Cardiac output rises. The peripheral blood vessels dilate to dissipate heat, reducing systemic vascular resistance. Blood flow to the skin and muscles increases substantially.
This hemodynamic stress — repeated regularly — appears to train the vascular system in ways analogous to exercise training. Endothelial function improves: the cells lining blood vessels become better at producing nitric oxide, which keeps vessels flexible and reduces atherosclerotic plaque development. Blood pressure falls in the hours following a sauna session, and chronic sauna users tend to have lower resting blood pressure and better arterial compliance than matched non-users.
The Heat Shock Protein Response
Beyond the cardiovascular effects, sauna use activates heat shock proteins — molecular chaperones that repair damaged and misfolded proteins within cells. Heat shock proteins have attracted significant research interest because of their role in cellular resilience, inflammation regulation and possibly longevity. They are activated by other forms of thermal stress but sauna use is one of the most effective and accessible triggers in humans.
Growth hormone is also released during sauna sessions. Studies have documented two to three-fold increases in growth hormone following a single sauna session, with the effect amplified by longer or more intense sessions. Growth hormone plays a role in muscle maintenance, fat metabolism and tissue repair — functions that are particularly relevant for older adults trying to preserve physical function.
Mental Health and Sleep Effects
Regular sauna users consistently report improvements in sleep quality, and there is a plausible mechanism: the sharp drop in core body temperature after leaving a hot sauna mimics the natural decline that signals to the brain that it is time to sleep. Sauna bathing in the late afternoon or early evening appears to advance the timing of sleep and increase slow-wave deep sleep duration. Several small trials have found improvements in mood and reductions in depression scores following regular sauna use, with researchers attributing this partly to endorphin release and partly to the social and meditative qualities of the practice in the cultures where it is most embedded.
