The Study That Reframed the Minimum
Physical activity recommendations have long cited 150 minutes per week of moderate-intensity activity as the target. For many people, particularly those who are older, dealing with chronic conditions or managing demanding schedules, 150 minutes feels like a significant commitment. The figure can read as all-or-nothing: if you cannot hit the target, why bother?
A 2022 analysis published in the British Journal of Sports Medicine — pooling data from 196 peer-reviewed studies covering nearly 30 million people — reframed the question. The researchers were trying to identify the dose-response relationship between physical activity and health outcomes: not what the optimal amount was, but what the minimum meaningful amount looked like. What they found was that 75 minutes of moderate-intensity activity per week — about 11 minutes per day — was sufficient to produce a statistically significant reduction in mortality risk, cardiovascular disease incidence and several cancer types.
What 11 Minutes Actually Achieves
Compared to being entirely sedentary, 11 minutes of brisk walking per day was associated with a 17 percent reduction in all-cause premature mortality risk. The risk of cardiovascular disease fell by 18 percent. Certain cancer types — colon, endometrial, breast — showed reductions in incidence of 11 to 17 percent. These are population-level associations, not guarantees, but they are consistent across a very large body of evidence and across multiple continents and demographic groups.
The mode of exercise matters less than the fact of movement. Brisk walking, cycling, swimming, dancing — anything that raises the heart rate to a moderate level produces comparable effects when equated for duration and intensity. The word "brisk" does matter: a leisurely stroll produces smaller physiological benefits than a pace that slightly elevates breathing and heart rate.
Why Even a Small Amount Works
The biology behind why short bouts of moderate activity produce meaningful health effects comes down to what happens to metabolically inactive muscle during those bouts. Even 10 minutes of walking activates lipoprotein lipase, improves post-meal glucose clearance, lowers circulating inflammatory markers and increases blood flow to organs that benefit from it — including the brain. The effect is not proportional: the first 11 minutes of daily movement produces disproportionately large health gains relative to the effort, because the comparison group is people doing nothing.
The dose-response curve for physical activity is steep at the low end and flatter at the high end. Going from zero to 11 minutes produces a larger per-minute health benefit than going from 60 minutes to 71 minutes. This is not an argument against doing more — more activity produces better outcomes up to quite high doses. It is an argument that the minimum meaningful threshold is much lower than most public health messaging implies.
The Practical Implications
Eleven minutes is short enough to be genuinely achievable for the large majority of people who currently do no structured physical activity. It can fit into a lunch break, a commute or an evening. The key behavioural insight from this research is that it should remove the all-or-nothing barrier. A person who walks for 11 minutes on a day they cannot manage a full workout has done something with measurable long-term health value, not nothing. That reframing matters for adherence over years and decades.
