The Mineral Behind 300 Biochemical Reactions

Magnesium is involved in more enzymatic processes than almost any other mineral in the human body — by most estimates around 300 to 600 distinct biochemical reactions depend on it, including protein synthesis, muscle and nerve function, blood glucose control, blood pressure regulation and the production of DNA and RNA. It is also essential for converting vitamin D into its active form in the body. Despite this, it remains one of the most commonly under-consumed minerals in Western diets, and its deficiency is rarely identified by standard clinical blood tests.

The reason standard testing misses most cases is technical: serum magnesium — the fraction measured in a routine blood draw — represents only about 1 percent of the body's total magnesium stores. The rest is in bones (about 60 percent) and within cells (about 39 percent). The body tightly regulates serum magnesium within a narrow range, pulling from bone stores if needed. By the time serum magnesium falls below the reference range, total body magnesium depletion is often severe. Many practitioners now use red blood cell magnesium or 24-hour urinary magnesium as more informative measures.

Symptoms That Are Often Attributed to Something Else

Muscle cramps, particularly nocturnal leg cramps, are among the most recognisable symptoms of low magnesium. Anxiety, irritability and difficulty sleeping are also consistent features — magnesium plays a key role in regulating the NMDA receptor system, which is involved in stress and cortisol response. Constipation, palpitations, fatigue and poor concentration round out a symptom profile that is extremely common but rarely attributed to nutrition in a clinical context.

Migraines have a particularly strong association with magnesium status. Multiple placebo-controlled trials have found that magnesium supplementation reduces the frequency and severity of migraines in people with documented deficiency, and some neurologists now recommend magnesium as a first-line preventative therapy before prescribing pharmaceuticals. The mechanism appears to involve magnesium's role in preventing cortical spreading depression — the wave of neuronal activity that initiates a migraine.

Why Deficiency Is So Common

Modern diets are low in magnesium for several reasons. Processing removes a large proportion of the magnesium from grains — white flour contains roughly 80 percent less magnesium than whole wheat flour. Soil depletion from intensive agriculture has reduced the magnesium content of vegetables over the past 50 to 70 years. Additionally, several common medications deplete magnesium: proton pump inhibitors (used for acid reflux), diuretics, and some antibiotics all reduce magnesium absorption or increase urinary losses. Alcohol consumption also increases magnesium excretion. Diabetes, which affects hundreds of millions of people globally, impairs magnesium reabsorption in the kidneys.

Food Sources and Supplementation

The richest dietary sources of magnesium are dark leafy greens (particularly spinach), pumpkin seeds, almonds, black beans, dark chocolate and whole grains. Adults generally require 310 to 420 mg per day depending on age and sex. For people who supplement, magnesium glycinate and magnesium malate are generally better absorbed and cause fewer gastrointestinal side effects than magnesium oxide, which is cheap but poorly bioavailable.
